Bone Grafting: What is Bone Grafting?
A bone graft is a surgical method where bone tissue is transplanted. Surgeons use this procedure to repair issues with bones or joints, helping to rebuild or fix damaged bones in areas like the hips, knees, spine, and sometimes other joints. Bone grafting is helpful in healing bones after injuries, certain fractures, and joint issues, or for encouraging bone growth around implanted devices, like in a total knee replacement (TKR).
The bone for a graft can come from your own body or a donor, or it can be artificially created. Once the body accepts the graft, it serves as a structure that helps the new, healthy bone grow.
The two main types of bone grafting are:
- Allograft: This type uses bone from a deceased donor. The tissue bank cleans and stores the bone. These tissue banks carefully screen, disinfect, and test the donated bone to ensure it is safe for use.
- Autograft: This type uses bone taken from the patient’s own body, often from areas like the ribs or hips.
In an allograft, the bone doesn’t come from your own body, so it doesn’t have any living cells and contains fewer growth-stimulating chemicals. The benefit of using an allograft is that you don’t need to donate your own bone, which shortens the surgery and may reduce pain afterward. Although allografts undergo thorough testing, a very small risk of transmitting infections remains.
Why Opt for Bone Grafting?
Bone grafting addresses several needs, including injury and disease. The four main reasons for bone grafting are:
- Fractures: Surgeons recommend it for complex or multiple fractures, or for fractures that don’t heal well after the initial treatment.
- Fusion: It helps two bones join and heal across a damaged joint, commonly in the spine.
- Regeneration: Bone grafting helps replace lost bone due to disease, infection, or injury, using small amounts for bone cavities or larger sections.
- Implanted devices: It aids in healing around surgical implants like joint replacements, plates, or screws.
Orthopedic procedures commonly use bone grafting in two key ways:
- To promote bone healing: It encourages the growth of new bone tissue.
- To provide structural support: It fills gaps between bones to strengthen the skeleton.
Stimulate Bone Healing
Bone grafting is used to encourage and accelerate bone healing. With an allograft, surgeons place the graft tissue near a fracture or fusion site. Chemicals within the graft, such as bone morphogenetic protein (BMP), stimulate the surrounding bone to heal faster. In an autograft, where the bone comes from your own body, the living bone cells (osteocytes) that survive the transfer continue producing new bone in the new location.
Provide Support to the Skeleton
Bone grafting also offers structural support by filling gaps between two bones with larger pieces of bone. Since bone is rigid, it helps keep the vertebrae separated while the body gradually grows new bone around the graft. Over time, the grafted bone will be replaced by your own new bone. The fusion process can be slow and may take several years, depending on the size of the graft.
The Risks of a Bone Graft
Like all surgeries, bone grafting comes with risks such as bleeding, infection, and reactions to anesthesia. Additional risks specific to bone grafting include:
- Pain
- Nerve injury
- Rejection of the graft
- Inflammation
Preparation for Bone Grafting
Your orthopedic physician will start with a thorough medical history and physical exam. They will ask about any medications, over-the-counter drugs, or supplements you’re taking.
If the surgery is not an emergency, you will likely need to fast before the procedure to avoid complications during anesthesia. It’s crucial to follow your doctor’s instructions carefully in the days leading up to and on the day of your surgery.
How a Bone Graft Is Performed
Your doctor will first discuss and decide on the best type of bone grafting for your situation.
You will receive general anesthesia before the surgery, which will put you into a deep sleep. An anesthesiologist will monitor your anesthesia and recovery throughout the procedure.
The surgeon will make an incision in the skin above the area where the graft is needed. They will shape the donated bone to fit the space and secure it using pins, plates, or screws.
Once the graft is in place, the surgeon will close the incision with stitches and bandage the area. A cast or splint will be applied to support the bone while it heals.
Recovery Process
The recovery process from a bone graft can vary based on the graft size and individual factors, typically spanning from two weeks to three months.
- Avoiding intense physical activity for up to six months may be necessary.
- To alleviate painful inflammation, icing and elevating the graft area can be beneficial.
- Adhering to your physician’s or therapist’s exercise recommendations is crucial to prevent muscle atrophy.
- Additionally, maintaining a healthy diet will support your healing process throughout the treatment.
